Job Description

Tasks & Accountabilities

  1. Undergo CITI training for biomedical researchers, clinical research associates, and good clinical practice (GCP).
  2. Travel to outreach and SCC as necessary to support patient referrals and follow-up care.
  3. Referral Management:
  4. Ensure referral forms are accurately developed and readily available in key locations.
  5. Identify & discuss potential patients with consultants for referral to SCC or outreach centers based on patients' home addresses.
  6. Collaborate with consultants to accurately complete referral forms with all necessary patient information.
  7. Submit referral forms for review and approval by the consultant.
  8. Share referral forms electronically with the referral centre on the same day of referral.
  9. Act as a point of contact for patients requiring referrals, ensuring they understand the process and expectations.
  10. Address patients' questions or concerns regarding the referral process.
  11. Keep patients informed about the status of their referrals.
  12. Collaboration with Healthcare Teams:
  13. Coordinate with consultants and healthcare professionals at the Hub & SCC to ensure efficient referral processing.
  14. Maintain strong relationships with outreach and SCC staff; identify and address socio-economic issues.
  15. Assist with collecting missing data for referred patients as needed.
  16. Administrative Tasks:
  17. Maintain accurate records of referrals and communications.
  18. Ensure timely completion of referral paperwork according to organizational guidelines.
  19. Assist with reporting and tracking referral outcomes.
  20. Quality Assurance:
  21. Monitor referral processes for quality and safety standards.
  22. Identify opportunities for process improvement and contribute to best practices development.
