We are looking to hire a Care Manager who will work with the team to:
- Ensure and supervise the client/patient to understand the prescription given or the counseling regarding lifestyle modifications if required by the doctor.
- Conduct holistic assessments (medical history, demographics, psychosocial history).
- Follow up with all new patients the next day via telephone regarding the quality of the first visit and resolve any issues if identified.
- Build care plans/rapport and identify support for clients with complex health issues through shared decision-making with clients and families.
- Assess for behavioral therapy needs and provide care if needed.
- Coordinate out-of-clinic specialist care with the consultant/FDO/nursing staff. Assist in consultant clinics if required.
- Assist in scheduling follow-up visits (day-to-day task).
Experience:
Bachelor's or master's degree in medicine, psychology, or public health with 2-3 years of experience in a healthcare setup.
