We are looking for an experienced and results-driven professional to lead, manage, and scale our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) operations within the U.S. healthcare domain. This leadership role requires strategic oversight of daily operations, client engagement, compliance adherence, team development, and performance analytics. The ideal candidate will bring deep expertise in CCM/RPM program workflows, CMS billing regulations, EMR platforms (e.g., eClinicalWorks, Athena, NextGen), and patient engagement strategies.

Title: Manager– Chronic Care Management (CCM) / Remote Patient Monitoring (RPM) Department: Operations / Revenue Cycle Management

Shift Timing: Evening (07:00pm-04:00am)

Location: Lahore

Key Responsibilities:

Program Leadership & Process Improvement

  • Oversee the end-to-end CCM and RPM operational workflow, including patient enrollment, monthly outreach, documentation, billing, and compliance readiness.
  • Develop, refine, and implement standard operating procedures (SOPs), call scripts, engagement strategies, and compliance frameworks.

Team Development & Management

  • Lead a multidisciplinary team comprising care coordinators, nurses (where applicable), and RPM support staff.
  • Drive team performance through structured hiring, onboarding, training, coaching, and quality assurance initiatives.
  • Set and monitor KPIs, ensuring productivity and adherence to quality benchmarks.

Client Relationship Management

  • Serve as the primary liaison for U.S.-based clients, managing onboarding, ongoing communication, performance reviews, escalations, and reporting.
  • Maintain high levels of client satisfaction and trust through proactive engagement and service excellence.

Compliance & Billing Oversight

  • Ensure operational alignment with CMS regulations and CPT codes including 99490, 99439, 99457, 99458, etc.
  • Supervise documentation practices to ensure accuracy and minimize billing errors or denials.

Technology & Integration Support

  • Collaborate with internal IT and third-party vendors to integrate RPM devices and data with EMR systems.
  • Ensure system-level compliance, data security, and workflow optimization.

Reporting & Analytics

  • Monitor key performance dashboards, including call logs, patient outreach metrics, billing accuracy, and reimbursement trends.
  • Generate analytical reports for internal leadership and clients, highlighting ROI, program growth, and engagement insights.

Required Qualifications:

  • Bachelor's degree in healthcare administration, Nursing, or a related field (Master's preferred).
  • Minimum of 5 years of experience in U.S. healthcare Revenue Cycle Management, with at least 2 years of focused experience in CCM/RPM operations.
  • Demonstrated success in scaling CCM/RPM programs (1,000+ patients/month).
  • Strong working knowledge of EMRs such as Athena, eClinicalWorks (ECW), NextGen, or Allscripts.
  • Expertise in HIPAA regulations, CMS billing guidelines, and patient consent processes.
  • Excellent leadership, communication, and client relationship management skills.

Preferred Qualifications:

  • Certified Professional Coder (CPC/CRC) or clinical credentials (e.g., RN, LPN).
  • Experience managing remote teams and coordinating across U.S. time zones.
  • Background in billing services supporting primary care or chronic disease management practices.
