Job Description
  • Clinical Pharmacist is responsible for ensuring the safe, appropriate, and cost-effective use of medicines to improve patient safety.
  • The Clinical Pharmacist will plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin (Managed By: IHHN).
  • Provide services across all medical and surgical specialties and participate in consultant rounds.
  • Monitor and assess patients by collecting specifications, conducting interviews, and evaluating patients to determine proper therapy.
  • Aim to improve medication compliance and coordinate with the multidisciplinary team to educate patients, attendants, doctors, nurses, and paramedical staff.
  • Implement medication reconciliation processes at admission to enhance patient safety.
  • Evaluate specific pharmacotherapy by assessing the appropriateness of drug therapy, its effectiveness, and identifying potential interactions or adverse effects.
  • Responsible for documenting detailed notes on patient admission, progress, and routine reviews in medical profiles.
  • Design comprehensive pharmacotherapy plans tailored to patient-specific problems, justified by pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based guidelines.
  • Provide drug information to healthcare professionals and document accordingly.
  • Participate in training pharmacists, pharmacy technicians, assistants, and trainees.
  • Maintain the hospital formulary and review non-formulary requests.
  • Monitor crash trolleys and floor stock monthly.
  • Conduct Continuing Medical Education (CME), Continuing Pharmacy Education (CPE), and webinars.
  • Prepare monthly reports on KPIs, interventions, expiry dates, near expiry, slow-moving, and dead stock.
