Diabetic Educator

Fresh graduate or up to 1 year of experience in diabetes/diet education or clinical care.

Registered Nurse (RN), Registered Dietitian (RD), or Bachelor's degree in nutrition, preferably with Certification in Diabetes Care and Education Specialist (CDCES).

1. Develop and implement individualized care plans based on patient's medical history, lifestyle, financial status, and diabetes management needs.
2. Conduct one-on-one and group education sessions on diabetes management, including medication adherence, insulin administration, and self-monitoring of blood glucose.
3. Provide education on nutrition, exercise, and lifestyle modifications to help patients achieve and maintain optimal blood glucose levels.
4. Coordinate care with other healthcare professionals, including endocrinologists, FM/DMP physicians, dietitians, and nurses.
5. Monitor patient progress through regular follow-up appointments and adjust care plans as needed.
6. Refer patients to emergency services if necessary.
7. Participate in community outreach activities to raise awareness about diabetes prevention and management.
8. Maintain accurate and detailed records of patient interactions, education sessions, and care plans.
9. Provide and utilize food models, booklets, SMBG forms, and aids to enhance patient's understanding.
10. Implement behavioral interventions to help patients achieve their self-management goals.
11. Counsel patients on regular SMBG, including glucometer use and recording readings, as well as medication timing and compliance.
12. Provide guidance on insulin use by addressing myths, demonstrating injection techniques, dose adjustments, proper site rotation, and ensuring timing and compliance.
13. Perform other tasks as required to support the Diabetes program.
14. Any task assigned by the direct supervisor and as decided by management.
