Clinical Nurse Coordinators are essential to our organization, providing exceptional patient care and ensuring seamless delivery of services. We are seeking a skilled Clinical Nurse Coordinator to join our team in Outreach Services.
Job DescriptionWe are looking for a highly motivated and organized Clinical Nurse Coordinator to provide coordinated care to patients in their homes. The successful candidate will have a strong background in nursing, excellent communication skills, and the ability to work effectively in a dynamic environment.
- Coordinate all aspects of patient care, ensuring timely delivery of services.
- Manage scheduling and staffing of individual cohorts.
- Conduct home visits, provide education, and review documentation.
- Participate in multidisciplinary rounds and plan patient care accordingly.
- Act as a clinical resource person for staff and conduct teaching sessions as needed.
To be successful in this role, you will need:
- A Baccalaureate Degree in Nursing (BScN) from an accredited institution.
- Minimum three years of nursing experience.
- Valid license from the Pakistan Nursing Council.
- Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
- In-depth knowledge of health and safety measures.
- Good communication and interpersonal skills.
- Ability to work in shift duties.
