
Patient Care Coordinator

We are seeking a skilled and compassionate Nurse Navigator to join our team. This role plays a vital part in ensuring seamless patient referrals and comprehensive follow-up care.
The ideal candidate will possess strong communication and collaboration skills, as well as the ability to navigate complex healthcare systems.
Responsibilities:- Referral Management:
- Develop and maintain accurate referral forms, making them readily accessible in key locations to ensure smooth processes.
- IDentify and discuss potential patients with consultants to refer them to specialty clinics or outreach centers based on their home addresses, prioritizing those with the greatest needs.
- Collaborate closely with consultants to complete referral forms with all essential patient information, ensuring completeness and accuracy.
- Submit referral forms for review and approval, streamlining the process and minimizing delays.
- Share referral forms electronically with the referral center on the same day of referral, fostering efficient communication and coordination.
- Serve as a key point of contact for patients, explaining the referral process and addressing any questions or concerns they may have, providing reassurance and support throughout.
- Keep patients informed about the status of their referrals, maintaining transparency and open communication.
- Collaboration with Healthcare Teams:
- Work closely with consultants and healthcare professionals to ensure efficient referral processing, leveraging collective expertise and resources.
- Maintain strong relationships with staff at outreach centers and specialty clinics, addressing socio-economic issues and promoting holistic care.
- Assist with collecting missing data for referred patients as needed, helping to fill knowledge gaps and improve overall care.
- Administrative Tasks:
- Maintain accurate records of referrals and communications, ensuring compliance with regulatory requirements and hospital policies.
- Ensure timely completion of referral paperwork, streamlining processes and reducing administrative burdens.
- Support reporting and tracking of referral outcomes, analyzing data to inform quality improvement initiatives and optimize care delivery.
- Quality Assurance:
- Monitor referral processes for quality and safety standards, identifying areas for improvement and contributing to the development of best practices.
- Promote a culture of continuous learning and quality enhancement, driving excellence in patient care and service delivery.
