We are seeking a highly skilled Senior Home Healthcare Professional to join our team. This role plays a crucial part in delivering patient-centered care through outreach services.

Duties and Responsibilities
  • Coordinate patient care processes, ensuring timely delivery of core services.
  • Manage scheduling and staffing for individual cohorts.
  • Conduct patient education sessions and review documentation during home visits.
  • Participate in multidisciplinary rounds and develop patient care plans.
  • Act as a clinical resource for staff and patients, conducting teaching sessions when needed.
  • Provide remote assistance to staff working in home settings.
Requirements
  • Baccalaureate Degree in Nursing from an accredited school.
  • Minimum three years of nursing experience.
  • Valid license from the relevant nursing council.
  • Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
  • In-depth knowledge of health and safety measures.
  • Good communication and interpersonal skills.
  • Willingness to work in shift duties.
