Job Summary

We are seeking a skilled Clinical Nurse Coordinator to join our Home Health Services team.

This role is responsible for coordinating patient care, managing scheduling and staffing, and providing education and support to patients and staff.

  • Key Responsibilities:
  • Coordinate patient care processes to ensure timely delivery of core services.
  • Manage scheduling and staffing for individual cohorts.
  • Conduct home visits, provide patient education, and review documentation.
  • Participate in multidisciplinary rounds and plan patient care accordingly.
  • Act as a clinical resource person for staff and conduct teaching sessions as needed.
  • Provide remote assistance to staff working in home settings.
Requirements

To be successful in this role, you should have:

  • A Baccalaureate Degree in Nursing (BScN) from an accredited institution.
  • Minimum three years of nursing experience.
  • Valid license from the Pakistan Nursing Council.
  • Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, and I/V Cannulation.
  • In-depth knowledge of health and safety measures.
  • Good communication and interpersonal skills.
  • Willingness to work in shift duties.
