Home-Based Palliative Care Coordinator

The Aga Khan University Hospital invites applications for the position of Home-Based Palliative Care Coordinator.

Job Responsibilities:

  • Attend and follow-up calls/referrals from various sources, including healthcare professionals, patients, and families, to explain necessary information and assign patients according to physiotherapist workload/assigned area and execute their treatment plan.
  • Extract inpatient admitted patients and visit wards/wings to explain home physiotherapy services to potential patients. Meet with discharged patients who require home physiotherapy services and liaison with healthcare professionals, patients, and families to seek additional referrals.
  • Assess patients, develop/formulate treatment plans for home physiotherapy patients, and ensure achievement of treatment goals. Review and evaluate patient assessment and progress notes of all home physiotherapists.
  • Participate in appropriate training programs, tutorials, and training sessions at the main physiotherapy department and ensure compliance with home physiotherapy policies and quality standards. Keep abreast of any policy changes that may affect home physiotherapy and communicate the same in a timely manner to concerned physiotherapists.
  • Follow-up with physiotherapists on a daily basis about their visits, services given to patients, and address any concerns/issues and timely close incident reports. Ensure effective utilization of physiotherapists and strive for improving their efficiency and competency by guiding them on the preferred way of handling customers.
  • Contribute to achieving positive patient satisfaction scores by being polite, helpful, efficient, and focused on delivering excellent customer service.

Requirements:

  • Successful completion of Doctor of Physical Therapy (DPT) degree from a reputable university.
  • At least five years of related experience in physiotherapy and capable of following standard physiotherapy procedures and providing physiotherapy services.
  • Excellent interpersonal skills, particularly communication skills.
  • Ability to multi-task and effectively work in teams as well as independently.
  • Excellent problem-solving and troubleshooting skills.
  • Proficiency in the use of computer applications, particularly Microsoft Office package.
  • Flexibility and ability to schedule time effectively.
