Nurse Coordinator for Home Healthcare Services

The hospital offers outreach services to provide primary and secondary healthcare to patients in their homes.
Key Responsibilities:- Patient Care Coordination: Ensure timely delivery of core processes, manage patient scheduling and staff assignments.
- Patient Education: Provide education and review documentation during home visits.
- Multidisciplinary Collaboration: Participate in rounds and plan patient care accordingly.
- Staff Support: Act as a clinical resource person for staff, conduct teaching sessions as needed.
- Remote Assistance: Offer assistance to staff working in home settings.
- Nursing Degree: Baccalaureate Degree in Nursing (BScN) from an accredited school.
- Experience: Minimum three years of nursing experience.
- Licensure: Valid license from Pakistan Nursing Council.
- Certifications: Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
- Health and Safety: In-depth knowledge of health and safety measures.
- Communication Skills: Good communication and interpersonal skills.
- Flexibility: Ability to work in shift duties.
