Job Description: We are seeking a highly skilled Clinical Pharmacy Specialist to join our team. As a Clinical Pharmacy Specialist, you will play a critical role in providing safe, effective, and economical pharmacotherapy for patients. Your primary responsibilities will include establishing, coordinating, implementing, and providing all aspects of clinical pharmacy activities with an emphasis on therapeutic drug monitoring, clinical pathways, formulary management, pharmaco economics, and patient outcomes through detailed reporting systems.
Required Skills and Qualifications:
- Establishes and coordinates all aspects of clinical pharmacy activities with emphasis on therapeutic drug monitoring, clinical pathways, formulary management, pharmaco economics, and patient outcomes.
- Assists in developing operational systems and procedures for the Pharmacy Department's Clinical Division that are in accordance with the guidelines of the Joint Commission on the Accreditation of Healthcare Organizations, and state hospital codes.
- Collaborates with physicians in establishing drug criteria for the safe, economical, and appropriate use of medications.
- Participates in multidisciplinary team rounds and provides pharmacotherapy recommendations during the discussions.
- Develops disease/drug therapy management programs to ensure safe, appropriate, and cost-effective use of medications.

Others: The selected candidate will be responsible for managing non-formulary requests for drugs, participating in Pharmacy & Therapeutics Committee activities, performing initial assessments of drugs having pre-established criteria to be monitored, and monitoring these drugs for follow-ups.
