Job DescriptionThe pharmacist will ensure proper handover and takeover procedures, documenting details in the register as per protocols.
Responsibilities include timely dispensing and distribution of medication, managing patient profiles, utilizing CPOE for rational drug use, preventing drug interactions, and promoting rational drug use.
Contact physicians to clarify any inaccuracies in medication orders and document interventions accordingly. If interventions are rejected, communicate with the clinical team for formulary committee review.
In case of medication non-availability, contact physicians for alternatives or process non-formulary arrangements according to NFI policy.
Ensure all orders are processed promptly and medications are delivered to the nursing units efficiently.
Monitor and manage inventory stock levels, including ordering and stock management as needed.
Prepare departmental requisitions for the warehouse and receive supplies, checking batch numbers, expiry dates, quantity, packaging, cold chain requirements, and proper shelving at specified temperatures.
Place light-sensitive, cytotoxic, high-alert, and look-alike/sound-alike drugs appropriately to prevent errors.
During OPD duties, dispense medicines directly to patients, counsel them, and respond to queries.
At oncology/cytotoxic stations, responsibilities include:
- Creating patient protocols by reviewing profiles and baseline labs (CBC, LFT, Creatinine) before chemotherapy, generating three copies of protocols (for nursing, patient, and pharmacy records).
- Double-checking and co-signing patient protocols.
- Verifying CPOE orders at chemo processing stations before drug preparation.
- Handling intrathecal drugs (Methotrexate, Cytarabine, Hydrocortisone) early morning to avoid errors, as per policy.
- Reviewing patient profiles for correct diagnosis, age, and protocol phase. Once the chemo regimen is prepared, each drug is double-checked and co-signed.
- Handover of chemo drugs to nursing staff for administration, documenting any non-administration or refunds in an Excel sheet as per protocol.
