Clinical Pharmacist

The Clinical Pharmacist is responsible for ensuring the safe, appropriate, and cost-effective use of medicines to improve patient safety.

The Clinical Pharmacist will plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin (Managed By: IHHN).

Responsibilities include providing services across all medical and surgical specialties and participating in consultant rounds.

Monitoring and assessing patients to collect specifications, interview, and identify proper therapy needs.

The aim is to improve medication compliance and coordinate with the multidisciplinary team in educating patients, attendants, doctors, nurses, and paramedical staff.

Achieving medication reconciliation at admission to enhance patient safety.

Evaluating specific pharmacotherapy by assessing the appropriateness of drug therapy, effectiveness, and potential interactions or adverse effects.

The Clinical Pharmacist will document detailed notes on admission, progress, and routine reviews of patient medical profiles.

Designing comprehensive pharmacotherapy plans tailored to patient-specific problems, justified by pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based guidelines.

Providing drug information to healthcare professionals and documenting accordingly.

Participating in training pharmacists, pharmacy technicians, assistants, and trainees.

Maintaining the hospital formulary and reviewing non-formulary requests.

Monitoring crash trolleys and floor stock on a monthly basis.

Conducting CMEs, CPEs, webinars, and preparing monthly reports on KPIs, interventions, expiry, near expiry, slow-moving, and dead stock.
