Coordinator, Home Based Palliative Care Hospital

The Aga Khan University Hospital invites applications for the position of Coordinator for Home Based Palliative Nursing.
ResponsibilitiesYou will be responsible to:
- Attend and follow-up calls/referrals from OSR, consultant clinic (CC), hotline number or request receive at HHS portal by explain necessary information to client, assign patients according to physiotherapist workload / assigned area and execute their treatment plan. In addition to this ensure to close request before CoB.
- Extract inpatient admitted patient and visit wards/wings to explain home physiotherapy services to potential patients. In addition to this meet with discharged patients which require home physiotherapy services and liaison with head nurses, inpatient physiotherapy coordinators, OSR, call centre and others to seek additional referrals.
- Assess the patients, develop / formulate treatment plan for home physiotherapy patients and ensure to achieve treatment goals. Review and evaluate patient assessment and progress notes of all home physiotherapist.
- Participate in appropriate in-house training programmes, tutorials, and training sessions at main physiotherapy department and ensure comply with the home physiotherapy policies and organizational quality standards i.e. JCIA and keep abreast of any policy changes in the organization which may affect home physiotherapy and communicate the same in a timely manner to the concerned physiotherapist.
- Follow-up with physiotherapist on daily basis about their visits, services given to patients and address any of their concerns/issues and timely close incident report (IR). Ensure effective utilization of physiotherapist and achieve budgeted volumes.
- Monitor physiotherapist performance and strive for improving their efficiency and competency by guiding them on the preferred way of handling customers.
- Contribute in achieving at least 90% positive indicator score in patients' satisfaction-related aspects by being polite, helpful, efficient with a focus on model and monitor departmental KPI.
You should have:
- Successful completion of DPT from reputable University.
- at least five years of related experience in physiotherapy and capable of following standard of physiotherapy procedure and ability to provide physiotherapy services.
- excellent interpersonal skills, particularly communication skills
- ability to multi-task and effectively work in teams as well as independently
- excellent problem solving and troubleshooting skills
- proficiency in the use of computer applications, particularly MS Office package
- Should be flexible and able to schedule time effectively
