• Ensure the safe, appropriate, and cost-effective use of medicines to improve patient safety.
• Plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin (Managed By: IHHN).
• Provide services to all medical and surgical specialties and participate in consultant rounds.
• Monitor and assess patients to collect specifications, conduct interviews, and identify proper therapy.
• Improve compliance and coordinate with the multidisciplinary team to educate patients, attendants, doctors, nurses, and paramedical staff.
• Implement medicine reconciliation processes on admission to enhance patient safety.
• Evaluate specific pharmacotherapy by assessing drug appropriateness, effectiveness, and potential interactions or adverse effects.
• Responsible for documenting patient reviews and notes in medical profiles.
• Design comprehensive pharmacotherapy plans tailored to patient-specific problems, justifying recommendations based on pharmacological, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based guidelines.
• Provide drug information to healthcare professionals and document accordingly.
• Participate in training pharmacists, pharmacy technicians, assistants, and trainees.
• Maintain hospital formulary and review non-formulary requests.
• Monitor crash trolleys and floor stock monthly.
• Prepare monthly reports on KPIs, interventions, expiry dates, near expiry items, slow-moving, and dead stock.
