Clinical Pharmacy Specialist

  • Develop and implement comprehensive clinical pharmacy services at a hospital setting.
  • Collaborate with medical teams to provide high-quality patient care.
  • Monitor patients, collect data, and interview patients to identify optimal treatment plans.
  • Enhance compliance by educating patients, attendants, doctors, nurses, and paramedical staff.
  • Implement efficient medicine reconciliation processes on admission to improve patient safety.
  • Evaluate pharmacotherapy by assessing appropriateness, effectiveness, and identifying potential interactions or adverse effects.
  • Document detailed notes on admission and progress.
  • Design personalized pharmacotherapy plans based on evidence-based information and standard practice guidelines.
  • Provide drug information to healthcare professionals.
  • Train pharmacists, technicians, assistants, and trainees.
  • Maintain hospital formularies and review non-formulary requests.
  • Monitor inventory levels and submit regular reports on stock levels.

Requirements: 3 years' experience in hospital pharmacy practice and clinical pharmacy practice. B. Pharmacy or Pharma-D from a recognized university.
