Senior Medication Management Specialist

At Indus-Hospital, we are seeking a highly skilled Clinical Pharmacist to join our team.

• Ensure the safe and effective use of medicines in improving patient safety.
• Plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin.
• Provide clinical pharmacy services to medical and surgical specialties and participate in consultant rounds.
• Monitor and assess patients to collect information, interview patients, and identify proper therapy.
• Improve compliance and coordinate with multidisciplinary teams to educate patients, attendants, doctors, nurses, and paramedical staff.
• Achieve medicine reconciliation processes on admission to enhance patient safety.
• Value specific pharmacotherapy by evaluating drug therapy, assessing effectiveness, and identifying potential interactions or adverse effects.
• Enter detailed notes on admission, progress notes, and routine notes when reviewing patient medical profiles.
• Design comprehensive pharmacotherapy plans for patient-specific problems, justify recommendations based on pharmacologic, pharmacokinetic, pharmacodynamic, pharmacogenomic, pharmacoeconomic, ethical, legal, and evidence-based information or standard practice guidelines.
• Provide drug information to healthcare professionals and document accordingly.
• Participate in the training of pharmacists, pharmacy technicians, pharmacy assistants, and trainees.
• Maintain the hospital formulary and review non-formulary requests.
• Monitor crash trollies, floor stock monthly.
• Prepare monthly reports of KPIs, interventions, expiry, near expiry, slow-moving, and dead stock.

• Master's degree in pharmacy from an accredited institution.
• Pharmacology license to practice in Pakistan.
• At least 2 years of experience as a clinical pharmacist in a hospital setting.
