Home Health Services Outreach Nurse

We are seeking a skilled Clinical Nurse Coordinator to join our team. As a key member of our healthcare organization, you will play a vital role in delivering patient care processes.
Responsibilities:- Ensure timely delivery of patient care services.
- Manage scheduling and staffing for individual cohorts.
- Conduct home visits to provide patient education and review documentation.
- Participate in multidisciplinary rounds to plan patient care.
- Act as a clinical resource person for staff and patients.
- Baccalaureate Degree in Nursing (BScN) from an accredited institution.
- Minimum three years of nursing experience.
- Valid license from the Pakistan Nursing Council.
- Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
- Profound knowledge of health and safety protocols.
- Excellent communication and interpersonal skills.
- Willingness to work in shift duties.
