Disease Management Specialist


About the Role

We are seeking a skilled Disease Management Specialist to join our team. In this role, you will develop and implement personalized care plans for patients with diabetes, taking into account their medical history, lifestyle, and financial status.


Main Responsibilities
  1. Care Plan Development: Design and implement individualized care plans that cater to each patient's unique needs, including medication adherence, insulin administration, and self-monitoring of blood glucose levels.
  2. Patient Education: Conduct one-on-one and group education sessions on diabetes management, providing patients with the knowledge and skills necessary to achieve optimal blood glucose levels.
  3. Nutrition and Lifestyle Guidance: Offer guidance on nutrition, exercise, and lifestyle modifications to help patients manage their diabetes effectively.
  4. Care Coordination: Collaborate with other healthcare professionals, such as endocrinologists, FM/DMP physicians, dietitians, and nurses, to ensure seamless care coordination.
  5. Patient Progress Monitoring: Regularly monitor patient progress through follow-up appointments and adjust care plans as needed.
  6. Emergency Situations: Refer patients to emergency services if necessary.
  7. Community Outreach: Participate in community outreach activities to raise awareness about diabetes prevention and management.
  8. Record Keeping: Maintain accurate and detailed records of patient interactions, education sessions, and care plans.
  9. Behavioral Interventions: Implement behavioral interventions to help patients achieve their self-management goals.
  10. MEDICATION EDUCATION: Counsel patients on regular SMBG, including glucometer use and recording readings, as well as medication timing and compliance.
  11. Insulin Use Guidance: Provide guidance on insulin use by addressing myths, demonstrating injection techniques, dose adjustments, proper site rotation, and ensuring timing and compliance.
  12. Additional Tasks: Perform other tasks as required to support the Diabetes program.
