### Job Description
As a Diabetes Educator, you will play a vital role in developing and implementing individualized care plans for patients with diabetes. This involves conducting one-on-one and group education sessions on diabetes management, including medication adherence, insulin administration, and self-monitoring of blood glucose.
### Key Responsibilities
* Develop and implement individualized care plans based on patient's medical history, lifestyle, financial status, and diabetes management needs.
* Conduct one-on-one and group education sessions on diabetes management, including medication adherence, insulin administration, and self-monitoring of blood glucose.
* Provide education on nutrition, exercise, and lifestyle modifications to help patients achieve and maintain optimal blood glucose levels.
* Coordinate care with other healthcare professionals, including endocrinologists, physicians, dietitians, and nurses.
* Monitor patient progress through regular follow-up appointments and adjust care plans as needed.
* Refer patients to emergency services if necessary.
* Participate in community outreach activities to raise awareness about diabetes prevention and management.
* Maintain accurate and detailed records of patient interactions, education sessions, and care plans.
* Implement behavioral interventions to help patients achieve their self-management goals.
* Counsel patients on regular self-monitoring of blood glucose, including glucometer use and recording readings, as well as medication timing and compliance.
* Provide guidance on insulin use by addressing myths, demonstrating injection techniques, dose adjustments, proper site rotation, and ensuring timing and compliance.
### Required Skills and Qualifications
* Registered Nurse (RN), Registered Dietitian (RD), or Bachelor's degree in nutrition, preferably with Certification in Diabetes Care and Education Specialist (CDCES).
