The Aga Khan University Hospital invites applications for the position of Coordinator for Home Based Palliative Nursing.

Responsibilities
  • Delivering Quality Care: Attend and follow-up calls/referrals from various sources, ensuring patients receive necessary information. Assign patients according to physiotherapist workload/area and execute their treatment plans in a timely manner.
  • Patient Acquisition: Identify potential patients through ward visits and meet with discharged patients requiring home physiotherapy services. Liaise with head nurses, physiotherapy coordinators, and other departments to seek additional referrals.
  • Treatment Planning: Assess patients, develop treatment plans for home physiotherapy patients, and ensure achievement of treatment goals. Review patient assessments and progress notes of all home physiotherapists.
  • Professional Development: Participate in in-house training programs, tutorials, and sessions at the main physiotherapy department. Ensure compliance with home physiotherapy policies and organizational quality standards.
  • Physiotherapist Support: Follow up with physiotherapists on daily basis regarding their visits, services provided to patients, and address any concerns/issues. Timely close incident reports and optimize physiotherapist utilization.
  • Performance Improvement:
  • Monitor physiotherapist performance and strive for improving their efficiency and competency by guiding them on preferred customer handling practices.
  • Contribute to achieving high positive indicator scores in patient satisfaction aspects by being polite, helpful, efficient, and focused on model KPIs.
Requirements
  • Qualifications: Successful completion of DPT from a reputable university.
  • Experience: At least five years of related experience in physiotherapy and ability to provide physiotherapy services following standard procedures.
  • Skills: Excellent interpersonal skills, particularly communication skills; ability to multi-task and effectively work in teams independently.
  • Problem Solving: Excellent problem-solving and troubleshooting skills.
  • Computer Proficiency: Proficiency in the use of computer applications, particularly MS Office package.
  • Flexibility: Ability to be flexible and schedule time effectively.
