Role Description
">This role offers an excellent opportunity to make a meaningful difference in the lives of patients and families. As a Nurse Navigator in Home Based Palliative Care Outreach Services, you will play a vital part in developing and implementing care plans, conducting patient assessments, and providing education on diagnosis, treatment options, and post-treatment care.
">- You will meet with newly diagnosed patients to discuss treatment plans and assess their understanding of these plans.
- Develop and use assessment tools to promote a consistent, holistic plan of care.
- Assess educational needs of patients, families, and caregivers, considering barriers to care such as literacy, language, and cultural influences.
- Provide education on diagnosis, treatment options, side effect management, and post-treatment care using various resources.
- Perform detailed counseling of patients and families before initiating care plans.
- Serve as the primary point of contact between the Home Health Team and outpatient/family members.
- Work closely with the research officer to maintain records of newly diagnosed patients and their outcomes.
- Participate in coordination of care plans with multidisciplinary teams.
