The role of the Home Health Care Navigator plays a vital part in delivering primary to secondary healthcare services to patients within their home environment.
Key aspects of the job include meeting with newly diagnosed patients after treatment plan formulation and reiterating the plan, assessing patient needs and understanding of the treatment plan, matching unmet needs with appropriate services and referrals, developing and using assessment tools to promote a consistent plan of care, providing education to patients, families, and caregivers about diagnosis, treatment options, and post-treatment care, supporting patients during periods of high emotional stress and anxiety, and serving as the primary point of contact between the Home Health Team and outpatient/family members.
