Work Shift: Onsite from3 pm till 12 am (or may shift to night from 6 pm to 3 am)

Location: Shahrah e Faisal, Karachi

EDUCATIONAL BACKGROUND: Bachelor in any Healthcare area.

KNOWLEDGE & EXPERIENCE REQUIRED: Fresh – 1 years max clinical/pharmacy experience or in the same setup as DataQ.

KEY RESPONSIBILITIES:

  • Identification of eligible patients for Chronic Care Management (CCM) services
  • Registry building /maintenance
  • Caseload Management which includes maintenance of reports
  • Providing accurate information to ensure maximal engagement with the patient by the Care Coordinator as per the standards set by DataQ Health which includes tracking outcomes of labs, referrals, PCP appointments & other treatments.
  • Provides support services for the care management team as needed
  • Works with the care management team to identify client/patient need for advocacy in the area of accessing health and behavioral health systems and services, entitlements, and community resources
  • Document all activities in the DataQ's CCM application following DataQ standards and identify trends and opportunities for improvement based on information obtained from interaction with patients and providers
  • Performs all other duties as assigned by the Supervisor of Chronic Care Management and Care Manager

SKILL SET & ABILITIES

  • Computer literacy and document management (Excel and e-files)
  • Ability and members of the health care team to build relationships with different types of people including the care team
  • Excellent verbal and written communication and interpersonal skills are required

Job Type: Full-time

Pay: From Rs60,000.00 per month
