Job Summary

The Claims Scrubber is responsible for reviewing, validating, and ensuring the accuracy of medical claims prior to submission. This role requires strong knowledge of billing guidelines, CPT and ICD coding, payer requirements, and eligibility verification processes. The Claims Scrubber plays a key role in minimizing denials, reducing rejections, and improving the efficiency of the revenue cycle.

Key Responsibilities

  • Review claims for completeness, accuracy, and compliance with payer-specific requirements before submission.
  • Verify correct usage of CPT, ICD-10, and HCPCS codes as per industry standards and payer policies.
  • Conduct eligibility and benefits verification to confirm patient coverage prior to claim submission.
  • Identify and correct claim errors (demographics, coding, modifiers, authorizations, etc.).
  • Flag and escalate recurring errors or payer-specific issues to the Billing Supervisor/RCM team.
  • Maintain up-to-date knowledge of payer guidelines, coding updates, and billing regulations.
  • Collaborate with AR and denial management teams to prevent repeat issues.
  • Ensure compliance with HIPAA and internal policies regarding patient data confidentiality.
  • Track scrubbed claims and generate reports on error trends, corrections, and improvements.
