- Provide accurate and relevant patient information regarding their medications, including dosage, side effects, and potential interactions.
- Ensure patients understand the importance of adherence to their medication regimen and answer any questions.
- Accurately interpret and process prescriptions from healthcare providers.
- Dispense medications promptly and efficiently, ensuring the correct medication, dosage, and quantity.
- Educate patients on the proper administration and storage of their medications.
- Review prescriptions for potential issues, such as drug interactions, allergies, or contraindications.
- Consult with Pharmacist to address concerns and make appropriate recommendations for alternative therapies when necessary.
- Document all interventions and communicate relevant information to the patient and healthcare provider.
- Ensure the pharmacy follows all relevant regulations, guidelines, and best practices to maintain patient safety.
- Regularly review and update pharmacy policies and procedures to minimize potential risks.
- Ensure quality improvement initiatives and collaborate with other healthcare professionals to enhance patient care.
- Accurately enter and maintain patients' prescription information in the pharmacy management system.
- Ensure proper posting / device entry for medications.
- Generate reports and analyse data to support decision-making and improve pharmacy operations.
- Monitor and maintain appropriate inventory levels of medications and supplies.
- Implement and oversee inventory control systems to minimize waste, prevent stockouts, and identify near-expiry or dead stock.
- Communicate inventory needs and concerns to the immediate supervisor and collaborate on solutions.
- Ensure proper medication placement, storage, and temperature control to maintain efficacy and safety.
- Receive incoming shipments of medicine and medical devices, verifying their accuracy and condition from warehouse
- Inspect packages for damaged and missing and report any discrepancies to the supervisor.
- Update inventory records and ensure proper documentation for received items.
- Ensure the pharmacy is clean, organised, and free from dust and clutter.
- Regularly inspect and maintain storage areas, shelves, and equipment to ensure they are in good working condition.
- Implement and enforce strict hygiene protocols to minimise the risk of contamination or infection.
- Analyse consumption trends and collaborate with healthcare providers to predict monthly medication demands.
- Prepare and submit accurate and timely demand forecasts to support efficient inventory management to warehouse for once in a month, keep sufficient stock to run pharmacy for a month without any interruption keeping exception for unexpected consumption or patient load.
- Timely notify the purchasing department of stock nearing to end fort the arrangement to avoid stock out condition.
- Assist in efficiently moving medical supplies within the warehouse using appropriate equipment, such as pallet jacks
- Follow standard operating procedures for loading, unloading, and transporting goods.
- Adhere to strict protocols when handling look-alike-sound-alike (LASA), high alert, flammable, and fragile products.
- Follow special handling instructions to prevent cross-contamination, mix-ups, or damage to sensitive items.
- Use personal protective equipment (PPE) when necessary.
- Conduct self-audits to verify stock both physically and electronically. Their batch expiry quantity is the same as in the software.
- Submit audit report to line manager, including physical condition, batch, expiry, quantity and proper storage conditions, mentioning near to end stock, dead stock, and non-running items in order to make effective use of them.
- Make report and share with Pharmacist, with evidence-based prescription trend if varies, increase consumption, or decrease consumption of particular medicine.
- Assistant in inventory management due to seasonal change for maintaining inventory forecasting demand.
- Return and replacement of any expired, broken, damage items or any other complaints, coordinating with supply chain to ensure smooth resolution.
- Filling and presenting KPIs to the Assistant manager and, Clinical Services
- Responsible to perform additional task and duties assign by management.
