TASKS & ACCOUNTABILITIES
- Undergo CITI training for biomedical researchers, clinical research associates, and good clinical practice (GCP).
- Travel to outreach and SCC as necessary to support patient referrals and follow-up care.
- Referral Management:
- Ensure referral forms are accurately developed and readily available in key locations once identified.
- Identify & discuss potential patients with consultants for referral to SCC or outreach centers based on patients' home addresses.
- Collaborate with consultants to complete referral forms with all necessary patient information.
- Submit referral forms for review and approval by the consultant.
- Share referral forms electronically with the referral centre on the same day of referral.
- Patient Support & Communication:
- Act as a point of contact for patients requiring referrals, explaining the process and setting expectations.
- Address patient questions or concerns about the referral process.
- Keep patients informed about the status of their referrals.
- Collaboration with Healthcare Teams:
- Coordinate with consultants and healthcare professionals at the Hub & SCC to ensure efficient processing of referrals.
- Maintain strong relationships with outreach and SCC staff, addressing socio-economic issues.
- Assist in collecting missing data for referred patients as needed.
- Administrative Tasks:
- Maintain accurate records of referrals and communications.
- Ensure timely completion of referral paperwork according to organizational guidelines.
- Support reporting and tracking of referral outcomes.
- Quality Assurance:
- Monitor referral processes for quality and safety standards.
- Identify improvement opportunities and contribute to best practices development.
