Job Responsibilities
  • Ensure the safe, appropriate, and cost-effective use of medicines to improve patient safety.
  • Plan, organize, and monitor the delivery of clinical pharmacy services at DHQ Hospital Badin (Managed By: IHHN).
  • Provide services across all medical and surgical specialties and participate in consultant rounds.
  • Monitor and assess patients to gather specifications, conduct interviews, and identify proper therapy.
  • Improve medication compliance and coordinate with multidisciplinary teams to educate patients, attendants, doctors, nurses, and paramedical staff.
  • Implement medication reconciliation processes at admission to enhance patient safety.
  • Evaluate pharmacotherapy by assessing drug appropriateness, effectiveness, and potential interactions or adverse effects.
  • Document patient reviews and notes accurately in medical profiles.
  • Design comprehensive pharmacotherapy plans tailored to patient-specific problems, justified by evidence-based guidelines and pharmacologic principles.
  • Provide drug information to healthcare professionals and document interactions accordingly.
  • Participate in training pharmacists, pharmacy technicians, assistants, and trainees.
  • Maintain and review the hospital formulary, including non-formulary requests.
  • Monitor crash trolleys and floor stock monthly.
  • Prepare monthly reports on KPIs, interventions, stock expiry, near expiry, slow-moving, and dead stock.
