Job Description

We are seeking a skilled Care Coordinator to join our team. The successful candidate will provide care coordination services to patients enrolled in chronic care management (CCM), principal care management (PCM), and remote patient monitoring (RPM) programs.

Key Responsibilities

  1. Provide comprehensive care coordination services, ensuring timely interventions and proactive communication with patients, caregivers, and healthcare providers.

  2. Conduct thorough telephonic assessments to develop personalized care plans that address the unique needs and goals of each patient.

  3. Monitor patients' health status, adherence to care plans, and progress towards desired outcomes, identifying potential barriers or gaps in care.

  4. Maintain open and effective communication with patients, caregivers, and healthcare providers to ensure seamless care transitions and optimize health outcomes.

  5. Educate patients about their chronic conditions, medications, preventive measures, and self-management skills, empowering them to take an active role in their healthcare.

  6. Collaborate with interdisciplinary care teams to coordinate and optimize patient care, leveraging resources and expertise to achieve better health outcomes.

  7. Document all care coordination activities and interventions in a comprehensive electronic health record system, maintaining accurate and up-to-date records.

  8. Ensure compliance with all applicable policies, procedures, regulations, and standards of care, upholding the highest standards of professionalism and integrity.

Requirements

Work Experience:

  • A minimum of 2 years of experience in telehealth care coordination is required, preferably in a US-based healthcare organization.

Education:

  • An MBBS degree along with a house job of 12 months is mandatory for this role.

Skills:

  • Critical Decision Making: Ability to make informed decisions quickly and effectively in high-pressure situations.
  • Interpersonal and Organizational Skills: Strong communication and interpersonal skills, with the ability to work collaboratively as part of a multidisciplinary team.
  • Data Entry and Management: Proficiency in accurately recording and managing patient data, using electronic health record systems and other relevant software.
  • Empathy and Patient-Centered Care: A genuine interest in delivering patient-centered care, with the ability to empathize with patients' needs and concerns.
  • Technical Skills: Proficiency in Microsoft Suite, including Outlook, Word, Excel, and experience working with EMR software.

Work Schedule: Monday - Friday, 6 pm – 3 am PKT (Night shift)
