Job Title: Advanced Practice Nurse Specialist

Clinical Nursing

We are seeking a skilled and compassionate clinical nurse to join our team as an Advanced Practice Nurse Specialist in the Home Health Programme. This is a unique opportunity to provide high-quality, patient-centered care to individuals in the comfort of their own homes.


About the Role

This position involves delivering comprehensive nursing care to patients with complex health needs. As an Advanced Practice Nurse Specialist, you will work closely with multidisciplinary teams to develop and implement individualized care plans that meet the unique needs of each patient.

You will be responsible for:

  • Providing expert nursing care to patients in their homes, including assessment, planning, implementation, and evaluation of care
  • Developing and implementing individualized care plans in consultation with patients, families, and healthcare teams
  • Conducting regular assessments to monitor patient progress and adjust care plans as needed
  • Collaborating with other healthcare professionals to ensure seamless care transitions and coordination
  • Documenting patient information accurately and thoroughly in electronic medical records

Requirements

To be successful in this role, you will need:

  • A Baccalaureate Degree in Nursing (BScN) or Post-RN BScN degree
  • At least six months of experience in a similar role, preferably in a home health setting
  • A valid license from the Pakistan Nursing Council
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team
