Home Health Services Coordinator

We are looking for a dedicated and experienced Clinical Nurse Coordinator to join our team.
This role is responsible for coordinating patient care, managing scheduling and staffing, and providing education and support to patients and staff.
- Key Responsibilities:
- Ensure efficient coordination of patient care processes.
- Develop and implement effective scheduling and staffing strategies.
- Conduct home visits and provide patient education.
- Participate in multidisciplinary rounds and contribute to patient care planning.
- Serve as a clinical resource person and provide training to staff.
- Offer remote support to staff working in home settings.
To succeed in this role, you will need:
- A Baccalaureate Degree in Nursing (BScN) from an accredited institution.
- At least three years of experience in nursing.
- A valid license from the Pakistan Nursing Council.
- Certification in CPR, Safe Medication Administration, Aseptic Techniques, and I/V Cannulation.
- Strong understanding of health and safety protocols.
- Excellent communication and interpersonal skills.
- Flexibility to work varied shifts.
