Palliative Care Services Lead

We are seeking a skilled professional to lead our Home Based Palliative Nursing initiative.
Key Responsibilities:- Attend and follow-up calls/referrals from various sources, assign patients to physiotherapists, and execute treatment plans while ensuring timely closure of requests.
- Extract inpatient information and visit wards to explain home physiotherapy services to potential patients, as well as meet with discharged patients who require ongoing care.
- Assess patients, develop personalized treatment plans, and review patient progress notes to ensure optimal outcomes.
- Participate in in-house training programs, tutorials, and sessions to maintain organizational quality standards and stay updated on policy changes affecting home physiotherapy.
- Monitor physiotherapist performance, address concerns, and close incident reports in a timely manner.
- A Doctor of Physical Therapy degree from a reputable institution is required.
- At least five years of related experience in physiotherapy, with the ability to adhere to standard procedures and provide high-quality services.
- Excellent interpersonal skills, particularly communication, and the ability to multitask and work effectively in teams.
- Proficiency in computer applications, especially MS Office, and flexibility to schedule time efficiently.
