Nursing Care Manager

As a Clinical Nurse Coordinator for Home Health Services, you will play a vital role in coordinating patient care processes and ensuring timely delivery of core services.
This position involves managing scheduling and staffing of individual cohorts, conducting rounds at patients' homes, providing patient education, and reviewing documentation. Additionally, you will participate in multidisciplinary rounds and plan patient care accordingly.
To be successful in this role, you must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a team environment.
Responsibilities- Co-ordinate patient care processes and ensure timely delivery of core services.
- Manage scheduling and staffing of individual cohorts.
- Conduct rounds at patients' home, provide patient education and review documentation.
- Participate in multidisciplinary rounds and plan patient care accordingly.
- Act as clinical resource person for staff and patients, conduct teaching sessions as needed.
- Provide remote assistance to staff working in home setting.
- Baccalaureate Degree in Nursing from an accredited school.
- Minimum three years of nursing experience.
- Valid license from Pakistan Nursing Council.
- Certification in Cardiopulmonary Resuscitation, Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
- In-depth knowledge of health and safety measures.
- Good communication and interpersonal skills.
- Willingness to work in shift duties.
