Job Title: Patient Care Specialist

We are seeking a skilled and compassionate Patient Care Specialist to join our team. In this role, you will be responsible for providing high-quality patient care and support in a fast-paced environment.

The ideal candidate will have experience working with patients, possess excellent communication skills, and be able to work independently as well as part of a team.

About the Role:
  • Job Summary: Provide patient care and support services to patients, families, and healthcare teams.
  • Responsibilities:
  • • Conduct patient assessments, develop and implement individualized care plans.
  • • Administer medications, perform basic lab tests, and provide wound care.
  • • Collaborate with interdisciplinary teams to coordinate patient care.
  • • Communicate effectively with patients, families, and healthcare professionals.
  • Requirements:
  • • A Pharm D / FSc / Matric Degree from an HEC-recognized university/board.
  • • Relevant experience in patient care or a related field.
  • • Excellent communication and interpersonal skills.
  • • Ability to work independently and as part of a team.
