Patient Care Coordinator

We are looking for a highly organized and detail-oriented individual to fill the role of Patient Care Coordinator. This position involves coordinating the delivery of patient care services in our acute care setting.

As a key member of our healthcare team, you will be responsible for ensuring that all aspects of patient care are coordinated effectively. This includes managing patient assignments, communicating with patients and families, and facilitating smooth transitions between departments.

  • Patient Assignment: Coordinate patient assignments with nursing staff to ensure efficient use of resources.
  • Communication: Maintain open lines of communication with patients, families, and healthcare teams to ensure seamless care coordination.
  • Care Coordination: Facilitate smooth transitions between departments, ensuring that all aspects of patient care are well-coordinated.
Qualifications and Experience
  • Education: Post RN B.Sc.N or B.Sc. General Nursing with at least 5 years' experience in an acute care setting.
  • Certifications: Should be registered with PNC and possess certifications in BLS, ACLS, and ECG. Additional diplomas in Health Management and specialization in ICU, ER are preferred.
  • Skills: Excellent organizational and communication skills, strong attention to detail, ability to work effectively in a fast-paced environment.
