Job DescriptionJOB SUMMARY:
Responsible for providing comprehensive clinical nutrition care services, including inpatient visitation, patient nutritional assessment, developing individualized nutrition plans, and instructing & educating patients & their families regarding disease-specific nutritional aspects. The candidate should possess knowledge of clinical nutrition care standards/principles and professional knowledge of tertiary care hospital-based clinical nutrition. The incumbent must ensure that documentation related to total nutritional care is appropriately maintained in a timely manner, in line with systems, processes, and nutrition policies. The individual should be trained to operate the Hospital Information System (HI) independently and be flexible to undertake and manage clinical nutrition operational activities during unusual situations or circumstances. The role also involves handling nutrition consults for inpatients and outpatients, maintaining professional coordination with faculty, physicians, nursing management, clinical nutrition department staff, and other allied healthcare providers.
TASKS & ACCOUNTABILITIES:
- Responsible for visitation, assessment, intervention, and multidisciplinary liaison related to assigned inpatient areas within 24 hours of patient admission, following credentialing criteria for performing clinical nutrition & dietetic consults.
- Prioritize nutrition care for patients referred by physicians, identify acuity levels, and complete nutrition assessments.
- Document patient nutrition assessments and participate in consultant and physician ward rounds as per TIH faculty schedule.
- Develop disease-specific nutrition care handouts in consultation with faculty. Conduct outpatient multidisciplinary nutrition clinics as approved and directed by the HOD, ensuring clinics are conducted by credentialed staff.
- Manage outpatient clinics (new and follow-up patients), ensuring timely work completion, proper use of tools/systems, and record maintenance.
- Ensure inpatients receive a healthy, balanced diet that is nutritionally appropriate and compliant with hospital standards.
- Develop, approve, and apply nutrition screening tools for patients at risk of malnutrition or with allergies, and conduct evaluations accordingly.
- Supervise and monitor dietetic trainees' daily activities to ensure quality nutritional care.
- Ensure therapeutic/non-therapeutic diets follow physician orders, considering patient preferences and allergies.
- Provide disease-specific nutritional education and guidelines to patients and families.
- Conduct monthly clinical nutrition training for diet technicians, dietitians, interns, residents, and allied health staff, maintaining records.
- Plan and budget for the annual trainee dietitian/intern program, ensuring implementation as per the HOD's directives.
- Coordinate post-meal ward rounds to ensure patient meals comply with physician diet orders.
- Adhere to professional and ethical standards outlined by the organization and relevant codes of conduct.
- Address patient complaints promptly, visiting patients as needed to ensure satisfaction.
- Participate in clinical nutrition activities during hospital emergencies and support roles as directed.
- Plan and manage the safe administration of enteral nutrition and TPN as per physician recommendations.
- Thoroughly evaluate all nutritional supplements/products prior to patient use, in consultation with faculty, pharmacy management, and supply chain.
- Perform any other tasks assigned by the HOD.
