Caregiver Coordinator Specialist

This role focuses on delivering exceptional client experiences by connecting clients with qualified caregivers. We are seeking a detail-oriented and compassionate professional to manage client and caregiver relationships.

The ideal candidate will possess strong organizational skills, attention to detail, and excellent communication abilities. They will be responsible for conducting comprehensive client intake calls, coordinating client-caregiver matching, and ensuring seamless service delivery and regulatory compliance.

Key responsibilities include:

  • Client Management & Onboarding:
    • Conduct thorough client assessments and initial evaluations
    • Create detailed client onboarding documentation
    • Coordinate client-caregiver pairing based on care needs and compatibility
    • Monitor client satisfaction and address concerns through regular follow-up calls
    • Manage client referrals and lead conversion processes from initial contact to service initiation
    • Respond to client inquiries with professionalism and empathy
    • Maintain accurate client records and service plans in company systems
    • Process service authorizations and ensure compliance with Illinois Department of Aging requirements
  • Caregiver Recruitment & Management:
    • Conduct thorough caregiver interviews via phone and video calls
    • Oversee the complete hiring process from application review to final onboarding
    • Verify caregiver credentials, certifications, and background check requirements
    • Develop and maintain detailed caregiver profiles and employment documentation
    • Coordinate caregiver scheduling and client assignments to optimize service delivery
    • Regularly check-in with active caregivers to ensure job satisfaction and performance
    • Address caregiver concerns and facilitate problem resolution
    • Track caregiver availability and manage schedule changes
  • Compliance & Documentation:
    • Ensure Illinois Department of Aging compliance requirements are met for all CCP clients
    • Verify all documentation meets regulatory standards and submission deadlines
    • Track caregiver certifications and license renewals to maintain compliance
    • Prepare and process hiring-related documents, including contracts and agreements
    • Generate professional documentation using PDF, Word, and DocuSign platforms
    • Organize digital filing systems for easy audit access
    • Produce compliance reports and track key performance indicators
    • Collaborate with the U.S. team on complex compliance issues
    • Monitor care plan adherence and document any deviations or concerns
    • Ensure HIPAA compliance in all client communications and documentation
