Clinical Nurse Care Coordinator

As a Clinical Nurse Coordinator, you will play a pivotal role in ensuring the timely delivery of high-quality patient care services. You will be responsible for coordinating patient care processes and managing scheduling and staffing for individual cohorts.
Responsibilities:- Coordinate patient care to ensure seamless service delivery.
- Manage patient flow and optimize resource allocation.
- Provide clinical guidance and support to staff and patients.
- Conduct home visits, provide education, and review documentation.
- Participate in multidisciplinary rounds and plan patient care accordingly.
- Baccalaureate Degree in Nursing from an accredited institution.
- Minimum three years of nursing experience.
- Valid license from a reputable nursing authority.
- Certification in CPR, Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
- Excellent communication and interpersonal skills.
- Ability to work in dynamic environments.
