Job Description:

We are seeking a dedicated and compassionate Youth Tracker to join our team. As a key member of our organization, you will be responsible for building strong relationships with youth, their families, and other stakeholders.

Your primary focus will be on completing daily contacts with youth, managing behaviors, and developing helping relationships. You will also be required to maintain accurate and timely documentation, including discharge information, and enter data into our tracking systems.

To succeed in this role, you must possess excellent verbal and writing skills, be able to manage conflict and crisis situations, and have the ability to assist with goal-oriented casework.

Essential Functions:
  • Daily Contacts: Conduct face-to-face interactions with youth at various locations, ensuring they receive the necessary support and guidance.
  • Behavior Management: Use programmatic and individual structure to manage behaviors and promote positive outcomes.
  • Documentation: Maintain accurate and timely documentation, including discharge information, and enter data into our tracking systems.
  • Communication: Develop and maintain effective communication with youth, their families, and other stakeholders.
Qualifications and Requirements:
  • A high school diploma or GED is required; a degree in a related field is an asset.
  • At least 3 months of experience working with youth and families or a related field.
  • A clean driving record, active driver's license, registration, and vehicle insurance are mandatory.
  • The ability to obtain First Aid Certification (CPR) is essential.
