Position Overview:

We are seeking a dedicated and detail-oriented
Specialized Care Programs Coordinator
to support our Chronic Care Management (CCM), Principal Care Management (PCM), and Behavioral Health Integration (BHI) programs. The coordinator will work closely with providers, patients, and care teams to ensure seamless delivery of care management services, improve patient outcomes, and maintain compliance with program requirements.

Key Responsibilities:

  • Coordinate and manage care for patients in Specialized care programs.
  • Conduct patient outreach calls for follow-ups, assessments, and care plan reviews.
  • Accurately document patient interactions in the EHR.
  • Monitor patient progress, address care gaps, and escalate issues when needed.
  • Facilitate communication between patients, providers, and care teams.
  • Ensure compliance with CMS guidelines and organizational policies.
  • Provide patient education on conditions, medications, and self-care.
  • Contribute to quality improvement and program performance reporting
  • Strong communication, organizational, and interpersonal skills.
  • Ability to multitask and work independently in a fast-paced environment.
  • Proficiency with EHR systems and Microsoft Office applications.

Full-time On-Site (5PM - 2AM)
