Key Responsibilities:
  • Coordinate patient care processes to ensure timely delivery.
  • Manage scheduling and staffing for individual cohorts.
  • Conduct home visits, provide education, and review documentation.
  • Participate in multidisciplinary rounds and plan patient care accordingly.
  • Act as a clinical resource person and conduct teaching sessions as needed.
  • Provide remote assistance to staff working in the home setting.
Requirements:
  • Baccalaureate Degree in Nursing from an accredited school.
  • Minimum three years of nursing experience.
  • Valid license from the relevant nursing council.
  • Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
  • In-depth knowledge of health and safety measures.
  • Good communication and interpersonal skills.
  • Willingness to work in shift duties.
