Clinical Nurse Coordinator Job Description

As a skilled Clinical Nurse Coordinator, you will be responsible for delivering high-quality patient care and coordinating timely delivery of core processes in an outreach services setting.

The successful candidate will work closely with the healthcare team to ensure seamless patient care and efficient operations.

This role is ideal for a detail-oriented and organized professional with strong communication skills and experience in nursing coordination.

  • Job Responsibilities:
  • Coordinate patient care processes and ensure timely delivery of all core processes.
  • Manage scheduling and staffing of individual cohorts.
  • Conduct rounds at patients' homes, provide patient education, and review documentation.
  • Participate in multidisciplinary rounds as applicable and plan patient care accordingly.
  • Act as a clinical resource person for staff and patients, and conduct teaching sessions as needed.
  • Provide remote assistance to staff working in home settings.
Requirements:
  • A Baccalaureate Degree in Nursing (BScN) from an accredited school.
  • Minimum three years of nursing experience.
  • Valid license from Pakistan Nursing Council.
  • Certification in Cardiopulmonary Resuscitation (CPR), Safe Medication Administration, Aseptic Techniques, I/V Cannulation.
  • In-depth knowledge of health and safety measures.
  • Good communication and interpersonal skills.
  • Willingness to work in shift duties.
